For part-time roles the salary and payments will be pro rata based on contracted hours.ResponsibilitiesAbout the role:Accept referrals assess need and make appropriate recommendations for care.Consult with Primary Care (GPs) community mental health services mental health hospitals IAPT services and other providers of mental health and substance use support to give services users a robust package of care to support their recovery. Be responsible for maintaining a full case load of service users with complex needs.Conduct comprehensive assessments construct care packages with multi-disciplinary partners where required review care plans in a structured format and manage onward referrals.Conduct home visits to service users to support those with disabilities and to those with family responsibilities.Develop and sustain links with multi-disciplinary including statutory and non-statutory providers Criminal Justice and wrap around services. About you:Excellent knowledge and understanding of drug and alcohol issues and experience of working within a related field.Excellent knowledge and understanding of prescription drug dependence related health and co-morbidities or ability and willingness to learn.Experience of working with clients with complex physical and/ or mental health needs including co-morbidity.Experience of working in a highly pressurised environment and the ability to use strategies to prioritise and manage work-load.Excellent team working and interpersonal skills maintaining a highly cooperative approach to supporting colleagues in delivering service objectives.What we will give to you:25 days holiday ( bank holidays) rising by 1 day for each years service Capped at 30 daysPaid Wellness hour each week along with a Wellness hub and Employee Assist ProgrammeContributory pension schemeSeveral benefits incl. discounts for shopping cinema holidays etc.A friendly and supportive teamTraining career development & progression opportunitiesPlease ensure that when completing your application form and supporting statement you reflect on the details outlined in the job description.
