Dementia Link Worker Vacancies at Dementia Concern

We provide specialist information, holistic advice and practical services in the community, and are commissioned by West London NHS Health Trust.

Dementia Link Workers -  35 hours per week – 2 Posts Available

Are you a self-motivated individual with health or social care experience to support people with dementia to live as independently as possible?  If so, you will be part of our team of Dementia Link Workers, in partnership with the NHS, providing excellent person-centred support to people living with dementia and their families/carers.

Applicants must have previous experience with vulnerable adults or older people.

In line with our safer recruitment practices, this post is subject to the Rehabilitation of Offenders Act (Exceptions Order) 1975 and as such it will be necessary for a submission for Disclosure to be made to the Disclosure and Barring Service (formerly known as CRB) to check for any previous criminal convictions.

Salary: £27,030.00

(moving to  £29,120.00 after 1 year)

Holiday Allocation: 25 days per year

Line managed by: Dementia Link Manager

Job Summary:

Dementia Link Service:

Dementia Concern is an Ealing Community Partner with the West London Health Trust (WLHT) and is the link for the person with dementia between secondary care, primary care and social services.

The Dementia Link Service responds to direct referrals from Cognitive Impairment Dementia Services (CIDS), liaising with GP practices. Each Dementia Link Worker will conduct reviews, assessments and referrals for people diagnosed with dementia. This role also informs and supports their families and any unpaid carers.

Dementia Link Workers (DLW) are office based, with extensive travel around the Borough and some home working, after successfully completing your probation period.

The Dementia Link Service is commissioned by the West London Health Trust and works to agreed targets. Dementia Concern actively seeks feedback from people with dementia and their families or carers to ensure the service meets their needs.

Key Responsibilities:                                                                               

The Dementia Link Worker will:

  1. Visit peoples’ homes to conduct client reviews and home assessments.
  2. Manage a caseload as a named worker for the person with dementia and their carer. This caseload will be overseen and monitored by the Dementia Link Manager.
  3. Review, assess and signpost refer to appropriate services.
  4. Provide relevant support and information to services involved in the persons care.
  5. Attend multi-disciplinary meetings, where required, to provide support and information regarding ongoing care, best interests and safeguarding.
  6. Support people with dementia and their families or carers, so that they can have choice and control over their current care plans and future care needs.
  7. Provide ongoing emotional support for people with dementia and their families or carers by phone cholinesteraseor home visits.
  8. Carry out reviews of the social, mental and physical health of people with dementia on cholinesterase inhibitors and update GPs following each review. 
  9. Refer back to Cognitive Impairment Dementia Services e.g. for medication issues or changing symptoms relating to dementia and inform the relevant health care professional.
  10. Keep detailed, accurate records of all actions undertaken.
  11. Attend appropriate support, supervision and training meetings relevant to this position.
  12. Promote awareness of dementia in the community through outreach and talks.
