With this role youll be working as part of the Sunderland Altogether Improving Lives (SAIL) Team in central Sunderland.The team supports people who may typically find it challenging to engage with treatment for drug and alcohol issues due to their complex needs. As a Complex Needs Recovery Coordinator you will support those people to engage with their Recovery Coordinator while working alongside other community services. Please see both of the attached job descriptions for details.Location: SunderlandHours: Full time 37.5 per weekFull Time Salary: 27861.26 - 32002.35 Dependent on experience (Based on full time hours pro rata for part time)Contract Type: Fixed term for 9 months from start dateFull-time hours at Change Grow Live are 37.5 hours per week. For part-time roles the salary and payments will be pro rata based on contracted hours.ResponsibilitiesAbout the role:Coordinate and leadasolution-basedapproach engaging Service Users inappropriate servicesCarry a caseload of complex clients developing personalised action plansParticipate in multi-agency initiatives to reduce the level riskWork closely with the drug and alcohol teamsto ensure that substance misusers and problem drinkers can accesstreatmentWork proactively to ensure that Service Users have access to ongoinghealthcareTo develop an Individual action Plan with each service user in conjunction with other agencies and professionals who will need to be involved.To work with identified complex needs clients with substance misuse and mental health issues and physical issues to overcome barriers to services and engagementAbout you:Experience of working in one or more of the following fields: substance misuse alcohol misuse mental healthExperience of working with people who have multiple and complexneedsKnowledge and experience of multi-agency workingwith experience offacilitatingmulti-agencymeetingsExcellent communicationnegotiationand advocacy skillsBe able to build andmaintainrelationships with a wide rangeofservicesCan work autonomously as well as part of ateamGood levels of IT literacy including the ability to use IT systems including MSOfficeapplicationsAble tocritically reflect and evaluate interventions service delivery and areasfordevelopmentWhat we will give to you:25 days of annual leave plus bank holidays.
