CRIMINAL JUSTICE LIAISON PRACTITIONERWELLBEING HUB, NOTTINGHAM, 12 MONTHS FIXED, 37HRS,A4C BAND 5, £31,049 - £37,796,FTE About the Role We are seeking a dedicated and motivated Criminal Justice Liaison Practitioner to join our Criminal Justice Substance Use Service. This is an exciting opportunity to play a vital role in bridging the gap between substance use services and Probation, ensuring that individuals on probation receive timely and effective support. In this role, you will act as the key point of contact between our service and Probation, facilitating effective communication, case coordination, and joint working. You will ensure that individuals referred via Probation are able to access assessment, treatment, and ongoing recovery support without delay. Key Responsibilities include: Acting as the primary liaison between the Criminal Justice Substance Use Service and Probation. Coordinating referrals, assessments, and care pathways for individuals with substance use needs who are on probation. Working collaboratively with Probation Practitioners to support care planning, risk management, and progress monitoring. Promoting a seamless and consistent approach to service delivery, shared decision making, and positive outcomes. Contributing to reduced reoffending by improving access to treatment, recovery support, and wraparound interventions that enhance health and wellbeing. We are looking for someone who is: Passionate about supporting vulnerable individuals to make positive life changes. Confident working in partnership with external agencies and professionals. Skilled in communication, problem solving, and case coordination. Able to work independently and manage a varied caseload. Experience within criminal justice, substance use services, or a related support setting is desirable.
