Disabled Children’s Service Targeted Support is looking for two social care assessors.

As a Social Care Assessor in Disabled Children’s Service Targeted Support, you will play a crucial role in assessing the needs of children and their families playing a key role in identifying their needs and support which may promote their outcomes.

Your responsibilities will include:

Conducting thorough assessments of children and families’ physical, emotional, and social needs. Developing personalized care plans to meet individual requirements which are co-designed with families and young people. Collaborating with children/ young people, families, and healthcare and education professionals to ensure the best possible outcomes. Monitoring and reviewing care plans to make necessary adjustments in order to promote a positive future. Providing support, guidance, and information to children/ young people and their families. Maintaining accurate records and reports.

Although understanding of disability and the challenges this brings for individuals and families is helpful, it is not essential. We will provide a robust induction and support.
