What will you be doing?
Implementing a new approach to support people who are using lots of unplanned services but no one service has been able meet their needs Direct work, at home or in the community, with adults whose non-medical needs cannot be met by frequent attendances at an Emergency Department Starting with a What Matters to You conversation, offering a personalised care approach, responding to an individuals priorities and needs Linking with and bringing in other support as appropriate
About you
You will have a good understanding of health inequalities and the wider determinants of health You will have significant experience as a case holding practitioner/worker, with an approach that aligns with personalised care You will have a coaching qualification, or have trained in motivational interviewing, appreciative enquiry or similar You will have experience of partnership working and navigating services
