Social Work Assistant post within the Integrated Care Assessment Team based at the Jeanie Deans in Helensburgh. Role involves undertaking assessments of need for community care packages (including financial assessments) and assessments for care home placements, reviewing care packages and maintaining accurate records following an individual’s discharge home from hospital.
