Job Duties:
As an Adult Care Services occupational therapist, you will hold your own caseload and work in partnership with people living in the community across Hertfordshire. You will undertake a holistic assessment of need to identify person-centred outcomes.
We work in an enabling way with people and their families and carers, to promote independence and participation, through meaningful occupation and a strengths-based approach. Across all our services we aim to ensure that people are provided with the right support and advice, and at the right time.
Our Adult Care Services practice principles are very much aligned to core occupational therapy values, with the ultimate aim of connecting people to the life that they want to live
We work collaboratively alongside social care colleagues, as well as with our health partners, housing and care providers, and a range of community support services.
Your role will also include an opportunity to mentor and develop others.
