We are currently looking for a highly motivated dynamic individual to join Torfaen Social Care and Housing Adult Services in the Learning Disabilities Team. Our aim is to work proactively with individuals, to promote choice, control, positive risk taking in order to achieve positive outcomes.

The successful candidate will be a qualified social worker keen to work in a way which focuses on 'what matters' to the individual and is able to form and develop relationships with individuals and carers to ensure that they are supported to achieve outcomes that matter to them.

The successful individual will work as part of a multi-disciplinary team comprising social workers, community nurses, an OT, a psychiatrist and a psychologist.

Principal Accountabilities and Responsibilities

  • To perform all the functions of the post consistent with legislation and guidance and within the policies, strategic aims, plans, standards, procedures and guidelines established by the Chief Officer of Social Care & Housing Services.
  • To support the Team Lead and be responsible for case management within Adult Services.
  • To promote and encourage best practice in order to deliver outcomes that matter to people and communities through collaborative conversations that identify strengths and assets and enable people to find their own solutions.
  • To develop, implement, review and quality assure plans to meet user needs and to ensure that services meet quality requirements and standards.
  • To ensure that the teams are flexible and responsive to changing needs and supported to prioritise appropriately.
  • To ensure teams safeguard adults at risk in line with statutory procedures and guidance
  • To identify opportunities for improvements in practice and performance through employing quality assurance mechanism and audit.
  • To develop effective relationships with all partners and stakeholders building links with community groups, developing relationships with other agencies and supporting to identify areas of unmet need.
  • To use IT systems to utilise information, measures and criteria to evaluate the functioning of the team and to ensure that such management information is produced regularly to enable service delivery to be monitored and effectively managed and to reported to Team Lead
