Job Purpose
To develop and deliver high quality social work services in relation to all aspects of the assessment and care management including the duty role of Children Service process – as relevant to the client group and in accordance with statutory and service area requirements.
Principal Accountabilities and Responsibilities
To perform all the functions of the post to comply with legislation and within the policies, aims and guidelines of the Social Services Department.
Prepare for, and work with individuals, families & others to assess their needs and circumstances.
Act appropriately upon receiving referrals/new cases, including reviewing case notes and other relevant material prior to making initial contact.
Work with the service user and others (family, carers, support groups and other professionals) as appropriate, to assess need and agree a course of action, using the approved model within the specialist area.
Develop care plans with service users and others to effectively meet identified needs in a positive and empowering way, maximise and work within the resource constraints of the service.
Demonstrate effective interagency, multidisciplinary and partnership working to provide a best practice service
Plan, carry out, review and evaluate social work practice with individuals, families & others.
Ensure the implementation of agreed care/support plans in liaison with other agencies and service providers, regularly monitoring, reviewing and revising as appropriate.
Address behaviour to the service user, and others, in a constructive way in order to minimise risk of harm and prevent abuse.
Manage an individual case load and associated administrative tasks in an efficient and effective manner, using IT systems and software as appropriate.
Support individuals to represent their needs, views and circumstances
Provide clear and accurate reports to the standard and within timescales required in the specialist area of work.
Advocate for, and with service users, families and others.
Work with service users and others to select the best form of representation according to the circumstances and needs of the service user.
Manage risk to individuals, families, self and others
Assess and manage risks to individuals, families, self and others
Regularly monitor, re-assess, and manage risk to individuals, families and others
Take immediate and proportionate action to deal with/address any behaviour that presents a risk
Manage and be accountable, with supervision and support, for your own social work practice and other if commensurate with grade
Maintain accurate, up-to-date case records and files according to the requirements of the department’s policies on data protection, record-keeping and confidentiality
Plan, carry out, review and evaluate social work practice with service users, carers, providers and partners.
Disengage from work with service users and others in an appropriately and timely fashion.
