Main duties of the job

You will be expected to carry out clinical visits to patients in their own homes and residential homes under the direction of the registered nurse.

The successful applicant must be flexible, a good team worker, used to working under pressure, have excellent interpersonal and IT skills and be able to communicate effectively with patients, carers and other members of the MDT.

We are interested in hearing from both Assistant Practitioners and Nurse Associate's with proven clinical and communication skills to work as part of our friendly, supportive and innovative team. Previous experience in a community setting in the NHS is highly desirable but not essential.

The successful applicant will demonstrate the ability to utilise a range of skills and experience gained in clinical practice to assist the registered clinician to undertake comprehensive and holistic assessments.

Car driver and access to a car for work are essential
