Adult Social Worker needed for our Hospital Discharge to Assess Team based in Bracknell paying £32ph

We are looking for an Experienced Social Worker to work within our Busy Hospital Team within the Adult Social Care Team at Bracknell Forest.

The person will need to have good knowledge of Hospital Discharges D2A experience, ability to complete Mental Capacity Assessments, Safeguarding experience.

Must have a driving licence and car as working will be between Office and based at the Hospital.

Must be able to apply a strength-based approach, using the conversations model to connect people to their community/social network, and problem solve together.

You will be a social worker currently registered with Social Work England and will have obtained a social work qualification.
