About the Role

We are seeking a qualified Social Worker to join our Community Mental Health Team for Older Adults. As a Social Worker, you will play a vital role in supporting adults with dementia to live fulfilling lives.

Key Responsibilities
  • Manage a caseload, prioritizing workload effectively and receiving regular supervision and guidance.
  • Undertake duty role and safeguarding assessing officer responsibilities.
  • Work as part of a multi-disciplinary team, connecting people and their families to community and social networks.
  • Take a strengths-based approach to support people with dementia to lead fulfilling lives.
  • Participate in supervision and appraisal, and take responsibility for own learning and development.

Requirements
