Home First Urgent Response Team - Social Worker

Pay rate - £35 an hour Umbrella / £27.50 PAYE

Swindon Borough Council

The position is within the Home first and Urgent Response Team. This is a busy and fast paced team and would require a dynamic worker with experience supporting hospital discharge discharge and admission avoidance. The team are based in the office 2 days a week, but expected to complete urgent visits at short notice. The suitable candidate must be a car driver and live within 45 mins of Swindon. Multi-disciplinary working is also essential to the role and evidence of this would be preferred.
