We are looking to welcome an approachable, enthusiastic, and knowledgeable, community-based Dementia Adviser to join our supportive and compassionate team, focusing on supporting people affected by dementia. Therefore, if you are interested, please submit your application as early as possible.About the roleLocation: Community based in North Wales with some homeworking elementsContract: 6 Months maternity coverWe work with 3rd sector partners as part of the North Wales Memory Support Pathway, the ideal candidate will be able to work within the areas of Conwy and Central North Wales. There is no direct care element to our role. We provide advice, support, guidance and problem solving help to people affected by dementia; passionately promoting their independence, improving sense of well-being, putting people in more control of their lives. This is sometimes by telephone, sometimes in person, and the role also involves building connections with other local services to help people find all of the support and activity available to them.This is a great opportunity to join our team providing personalised advice and support to people diagnosed with dementia and their carers and to promote our service and build relationships with a range of health and social care professionals in the voluntary and private sector.What you will do:

  • Offer a vital, compassionate advice service to support and guide those affected by dementia - in person and by phone.
  • Provide community based, face to face visits for an increased level of support for more complex cases.
  • Manage referrals, assist clients in assessing their information and support needs in a person-centred manner.
  • Problem solve - helping people find ways to better live around symptoms and challenges that come from their dementia.
  • Provide knowledgeable and trusted information regarding services and benefits available and, where appropriate, assist clients in accessing them.
  • Network local services to strengthen a Dementia Friendly Community.
  • Develop a proactive approach in reaching people with dementia and carers who may not otherwise access our services.
About you
  • Have a strong understanding of dementia.
  • Good organisational and time management skills.
  • An ability to assess client needs along with the understanding of the need for client confidentially.
  • Strong communication skills, both written and verbal.
  • An ability to manage and support challenging emotional situations.
  • Strong ICT skills.
The successful candidate must live within, or in reasonable distance from, the borders of North Wales. Travel expenses will only be claimable within North Wales (except for role requirements for travel elsewhere).
