An exciting opportunity has arisen for a Reablement Hospital Liaison Co-ordinator. The role is based at Hinchingbrooke Hospital but also allows for some homeworking based on the needs of the Service.

This is a fixed term position to the end of March 2025.

The role requires good knowledge of social and health pathways of support. Working as part of the Prevention Hub to triage referrals received from acute and community settings to determine eligibility for reablement pathway.

You will work alongside The Adult Early Help team who provide advice and support to anyone over the age of 18, who is seeking support or social care and will be part of our Prevention and Early Intervention services made up of Social Workers, Occupational Therapists, Adult Support Coordinators and Welfare Benefit Advisors from a variety of backgrounds. You will be part of the service offering support to individuals and to families and carers.

What will you be doing?

You will be working with individual referrals received from the multi-disciplinary partners to determine whether the reablement pathway is the most appropriate pathway for support in line with the Care Act 2014.

Inquisitive and creative practice is embraced, to allow for personalised solutions, taking into account wider community resources and organisations. The work will support in ensuring early and safe discharges from, but not limited to, Hinchingbrooke Hospital and Addenbrookes Hospital, to ensure correct discharge plans are in place.

You will support the knowledge of health colleagues complimenting them with your own, but also training them to develop their own knowledge of our Prevention and Early Intervention approach and of Adult Social Care as a whole.

About you

You will have a good understanding of all relevant legislation to a level where you can confidently challenge health colleagues where required. You will also have experience of assessing needs and providing support. A good knowledge of the health system and the support it offers is important.

You will need to have good and accurate literacy, numeracy and administrative skills and the ability to analyse information to solve general queries.

It is imperative that you have strong communication skills, a confident telephone manner and can work on your own initiative.

Good planning and organisation skills will be key, and you should be used to working under pressure to meet deadlines, allowing flexibility and resilience.

You will need a good working knowledge of the Care Act 2014 and Mental Capacity Act 2005 and experience of multi-disciplinary working to enable best outcomes for individuals.
