We are seeking a highly skilled and compassionate Locum Band 6 Specialist Community Neurological Occupational Therapist to join our team in providing specialized occupational therapy services to patients with long-term neurological conditions in the community.
Working 3 days a week ideally Mon/Tue/Wed but can be flexible. Will need current experience of providing occupational therapy planning, assessment and rehabilitation in the community for adults with complex long term neurological conditions. This includes people with MS, MND, Parkinson’s, stroke, FND and other neurological conditions. Must be able to undertake skilled and specialist assessments to formulate individualised management and treatment plans using evidence based clinical reasoning skills and utilising a wide range of treatment skills and options. Candidates will need to have a car as they will be visiting patients in their own homes and at clinic locations
The successful candidate will play a pivotal role in supporting patients' independence, enhancing their quality of life, and promoting their participation in meaningful activities despite the challenges posed by their neurological conditions. This role involves comprehensive assessments, goal setting, and therapeutic interventions aimed at optimizing patients' functional abilities and well-being within their home and community environments.
Key Responsibilities
* Conduct holistic occupational therapy assessments of patients with long-term neurological conditions, including stroke, multiple sclerosis, Parkinson's disease, and traumatic brain injury, to evaluate their physical, cognitive, emotional, and environmental needs.
* Collaborate with patients, their families, and other healthcare professionals to develop individualized occupational therapy care plans and goals that address patients' functional limitations, promote independence in activities of daily living (ADLs), and enhance their participation in meaningful activities.
* Provide direct occupational therapy interventions and rehabilitation programs to patients within their home and community environments, focusing on activities such as mobility training, upper limb rehabilitation, cognitive retraining, and adaptive equipment prescription.
* Implement evidence-based therapeutic approaches and techniques to address patients' specific impairments and challenges related to their neurological conditions, promoting neuroplasticity, compensatory strategies, and skill acquisition.
* Conduct home visits and environmental assessments to identify barriers to independence and safety within patients' home environments, recommending modifications, adaptations, and assistive devices to enhance accessibility and facilitate optimal function.
* Collaborate closely with other members of the multidisciplinary team, including physiotherapists, speech and language therapists, nurses, and social workers, to coordinate integrated care plans, address interdisciplinary goals, and optimize patient outcomes.
* Provide education and training to patients and their caregivers regarding strategies for managing symptoms, conserving energy, preventing complications, and maximizing functional independence in daily activities.
* Monitor patients' progress and outcomes over time, reassessing goals and treatment plans as needed to reflect changes in patients' abilities, needs, and goals, and adjusting interventions accordingly.
* Document all occupational therapy assessments, interventions, and progress notes accurately and comprehensively in electronic health records, ensuring compliance with professional and organizational standards and maintaining patient confidentiality.
* Engage in continuous professional development activities to enhance clinical knowledge, skills, and competencies in neurological occupational therapy, staying abreast of current research findings, best practices, and emerging interventions.
* Adhere to professional standards, codes of conduct, and regulatory requirements governing occupational therapy practice, including safeguarding vulnerable adults and children, and maintaining professional boundaries.
Qualifications & Experience
* Bachelor's or Master's degree in Occupational Therapy from a recognized institution.
* Registration with the Health and Care Professions Council (HCPC) as an Occupational Therapist.
* Minimum of three years of post-qualification experience in occupational therapy, with specific experience working with patients with long-term neurological conditions in the community setting.
* Specialist knowledge and skills in neurological rehabilitation, including assessment, intervention, and goal setting for patients with diverse neurological diagnoses.
* Excellent communication and interpersonal skills, with the ability to build rapport, establish effective relationships with patients and families, and collaborate with multidisciplinary teams.
* Ability to work autonomously and collaboratively within a community-based and multidisciplinary team environment, demonstrating flexibility, adaptability, and teamwork.
* Proficiency in using electronic health record systems and other relevant technology tools for documentation, communication, and information management.
* Commitment to providing patient-centered care, promoting independence, and advocating for the needs of patients with long-term neurological conditions and their families
