Are you a dedicated and experienced Qualified Social Worker /Senior Practitioner with a passion for supporting adolescents? Purpose of the Job: As a Qualified Social Worker /Senior Practitioner in the Adolescent Team, you will manage a varied caseload of adolescents, providing comprehensive assessments, planning, implementation, and evaluation of actions to promote their welfare. You will work collaboratively with multi-disciplinary teams to address the complex needs of young people, ensuring they receive the support and guidance necessary to thrive.
Key Responsibilities:
* Conducting thorough assessments of adolescents and their families
* Developing and implementing effective care plans tailored to individual needs
* Providing direct support and intervention to young people
* Collaborating with schools, healthcare professionals, and external agencies
* Maintaining accurate and detailed case records
* Advocating for the best interests and rights of adolescents
* Participating in team meetings, case discussions, and ongoing professional development

Requirements:
* Qualified Social Worker with current SWE registration
* Experience in children's social work, particularly with adolescents
* Strong assessment, planning, and organisational skills
* Excellent communication and interpersonal abilities
* Commitment to safeguarding and promoting the welfare of young people
* Ability to work effectively within a multi-disciplinary team
