OVERVIEW
Improve each client’s quality of life and promoting and maintaining independent living for themselves and any dependents they may have.
KEY RESPONSIBILITIES
• To provide intensive casework, support, and monitoring of general, and support needs, facilitated by key-sessions, community meetings, and general day-to-day contact.
• To provide advice, information, and support to the service users on safety – including around domestic abuse and independent living, as required.
• To work with service users in co-producing a ‘Safety Plan’ and ‘Strengths and Aspirations Plan’, with a focus on the individuals’ strengths to meet their chosen goals, around the move on process and establishing a new home.
• To fulfil our obligations and responsibilities to inform, and work with any relevant statutory bodies, for example, Social Services in regard to Child Safeguarding issues/concerns and to follow our client’s Safeguarding policies and procedures at all times.
• To provide intensive practical and emotional housing support to service users prior to and after moving on from the service.
• To help facilitate, promote, and evaluate social activities and workshops for service users which may focus on, and encourage empowerment, for example self-esteem and confidence building and assertiveness training.
• To work in line with our clients Approach” this approach has the principles of co-production and recovery at its centre.
• To develop links with and then work with a range of other agencies (including drug and alcohol services, foodbanks, mental health teams, this list is exhaustive) in order to both safeguard and promote the welfare and/or opportunities of our service users.
• Support and empower service users to develop skills to enable them and enhance their tenancy sustainment skills.
• To establish close contacts with the various communities and statutory and voluntary organisations in the borough supporting the service users settling into their new community settings.
To design and run workshops and training sessions to housing providers and RSL’s to improve system understanding of associated challenges.
