AKTON Recruitment is working closely with the local authorities in the Tower Hamlet area. We are looking Targeted Youth Support Case Worker / Youth Support Case Worker.
An exciting opportunity has arisen to join the Targeted Youth Support Team as an Targeted Youth Support Case worker. We are looking for a self-motivated individual who is looking for a new challenge.
The Targeted Youth Support Team is currently working with Children aged 11-19 years old (25 with SEND). The team made up of skilled and experienced practitioners aiming to coordinate and develop an evolving schedule of bespoke support designed to match the learning and emotional support needs of each young person as well as providing support to the wider family. The direct support to Young People may include mentoring, advocacy, information, advice and guidance, along with learning and development opportunities that aim to reduce vulnerability and risk taking as well as increasing self-reliance, resilience and self-worth. Support to the wider family will be dependent upon need with the Early Help Assessment used with the family to identify their needs, identify solutions and plan interventions.
Key Responsibilities
* Initiate and lead on the completion of assessments using the Early Help Assessment Framework where there is a range of concerns.
* Act as the Lead Professional
* Convene Team around the Family (TAF) meetings.
* Support Family Group Conferencing to enable families to work with their own strengths and resources to develop safe plans with their children.
* Ensure a more family focused approach where initial concerns are identified, ensuring that families are more engaged in seeking early solutions to emerging concerns.
* Support for designated safeguarding leads and other professionals in relation to safeguarding
vulnerable children.
* Bridge the gap in communication between early help providers, across different sectors, and the statutory teams, ensuring that only appropriate cases are referred for statutory assessment
Requirements
1. Minimum Level 3 qualification in education, social care, childcare or health services.
2. Be flexible with working hours and work from various locations.
3. Relevant qualification in, for example, education, training, guidance, counselling, youth and community work, health or social service work.
3. Experience of face-to-face work with young people and families in a formal or informal setting (11- 19 and up to 25 for SEND).
4. A thorough grasp of the issues affecting young people aged 11-19, particularly the impact of social and economic disadvantage on young people's motivation, confidence and progress through education, employment and training
