The service is currently looking for RMNs and Mental Health Social Workers for a contract position working as a Band 6 Social Worker. The post holder will be supporting individuals under the age of 65 with a diagnosis of dementia. Mental Health experience is essential, also with a valid driver’s license.
Purpose of the service;
* Work as a member of the wider support team.
* Help in the continuing development of a high-quality service that is underpinned by evidence-based practice, care pathways / packages.
Duties will involve;
* Providing timely documentation.
* Conducting various health-based assessments.
* Risk assessments.
* Attending MDT meetings.
* Physical health checks.
* Personalised care.
This position is full-time hours – working 37.5 hours per week Monday to Friday (9 to 5).
Competitive Pay rates.
The applicant must be experienced in mental health and able to work at Band 6 level.
