Job summary
This role is part of a key worker pilot designed inpartnership with NHS England. The pilot is expanding its current age range upto 25 years old. We will continue to deliver a new way of supporting Children,Young People (CYP) and Young Adults up to the age of 25 years with AutisticSpectrum Disorders (ASD) and/or a Learning Disability (LD) and theirfamily/carers. This role will enable Children, Young People (CYP) and Young Adultsand their family/carers to get the right support at the right time and ensurelocal systems are responsive to meeting their needs in a holistic and joined upway.
The pilot is part of the NHS Long Term Plan which aspires forall CYP with ASD, LD or both, in an inpatient setting, to have a namedkeyworker by 2024. Devon is one of 13 national key working pilot sitesimplementing this strategic role.
This role will work across education, health, and social careto enable whole system to change and embed principles of transforming care andSEND reforms to improve the experience of CYP and Young Adults and families/carers.

Main duties of the job
The principal purpose of the job is toimprove the health & wellbeing of children, young people, young adults, andtheir families/carers with a range of mental health difficulties. This will beachieved by:
Active involvement in the delivery of high quality, safe and compassionatemental health services as an effective member of the multi-disciplinary team.
Implementing specific assessment, review and care packages toindividuals and groups within standard protocols, integrated care pathways
Providing evidence-based advice and consultancy to youngpeople, family members/carers within standard protocols
Organising and managing a defined caseload under distantguidance and work as lone workers where appropriate.
The post holder will be accountablefor their own actions within the delivery of this care.
Seeking advice and expertise of a registeredpractitioner when required through the use of regular managerial and clinicalsupervision in accordance with Livewell policy.
