One of our local authority clients are currently recruiting for a Health Care Navigator. This is a 6 month temporary contract with possible further extension based on budget and performance.
Main Purpose of Post/Job Summary
Health Navigators play a crucial role in the resettlement of rough sleepers, as they require a specialist intervention and multi-disciplinary approach to secure sustainable outcomes. They will take a leading role to coordinate interventions, working with the existing Street Outreach Team and other existing partnerships.
Duties and Responsibilities
* To offer early intervention and ongoing support to prevent individuals with multiple and complex needs from returning to rough sleeping following discharge from hospital.
* To Assist rough sleepers to find appropriate mental health resources i.e.Caregivers, CPNs, Facilities and Services.
* Accompany individuals to appointments and help them to ensure an appropriate response whilst empowering them to plan their exit from the service and onto sustainable ways of living.
* To provide information of the best options and care available and where that may be.
* Involvement in assessments to help co-design a person centred, strength-based support plan according to their needs.
* To Provide intensive one-to-one support and consistent contact to an individual referred to the service pending their discharge from hospital;
* Define short and long term goals, form care plan and agree follow ups.
* Maintain communications with professionals and other relevant agencies
* Deliver better services for people with long term conditions which is proactive, holistic, preventative and person centred
* Use a whole system approach which is collaborative and personalised, linking inwith relevant professionals.
* Ensure the clients plays an active part in determining their support needs
* Provide continual support to ensure that the individual is resident in the accommodation that is appropriate to their needs, settled into their accommodation and they have access to and are engaging with primary / secondary care services to meet their healthcare needs.
* Adopt a multi-disciplinary approach with a focus on coordinating support.
* Make connections with a wide range of support services (including specialist services such as mental health support and substance misuse treatment) to facilitate support and assistance for individuals to address social and health issues and other personal barriers to sustaining housing.
* Identify and report back to the Specialist Team, any changes in services in the wider system which are needed to improve support given to the target cohort of customers.
* Record output and outcome monitoring in line with Team procedures.
