Key Responsibilities

You will provide mental health assessments, care planning, and interventions for older adults in the community, ensuring comprehensive and compassionate care.
*
Collaborate with the multi-disciplinary team to deliver integrated support and tailored care plans for older adult patients.
*
The role is Monday to Friday, with flexibility to work either 9:00-17:00 or 8:00-16:00, with weekend shifts available.
Position Details:
* Location: High Wycombe
* Work Pattern: Monday to Friday (9:00-17:00 or 8:00-16:00)
* Rates:
* Weekday Rate: £27.00 per hour
* Saturday Rate: £33.00 per hour
* Sunday Rate: £38.00 per hour
* Pay Type: Umbrella or PAYE available
Requirements:
* Registered Community Psychiatric Nurse (CPN) with experience in community settings and older adult care
* Flexibility to work weekdays with optional weekend shifts
