The primary purpose of the role is to deliver social work services aimed at safeguarding the welfare of children and young people, ensuring their protection in accordance with legal and statutory obligations. The position involves providing support to various groups of children, including those who are looked after, those with disabilities, and those in need of support during difficult times. The role also focuses on preventing harm, abuse, and reoffending among young people.
Key Responsibilities:
1. Assessment and Planning: Conducting Initial, Core, and specialized assessments to develop support plans and ensure appropriate service provision.
2. Case Management: Managing a range of cases, assessing risks, and coordinating support to service users and their families, while regularly monitoring and adjusting plans as needed.
3. Direct Family Support: Working closely with families to provide advice, promote self-sufficiency, and facilitate independence from social services.
4. Inter-agency Collaboration: Leading coordination with relevant staff and agencies, establishing necessary resources and services to meet the needs of service users.
5. Multidisciplinary Participation: Engaging in multidisciplinary teams and meetings to plan and review care programs for service users.
6. Documentation and Reporting: Ensuring accurate and timely recording of all relevant information and reports in compliance with policies and procedures.
7. Legal Responsibilities: Providing evidence in court or other panels when required, and implementing court-mandated actions to ensure the safety and well-being of service users.
8. Safety and Well-being: Ensuring the health, safety, and welfare of oneself and others by adhering to relevant policies and procedures.
9. Equal Opportunities Awareness: Demonstrating an understanding of equal opportunities and the diverse needs of service users.
10. Flexibility: Performing additional duties as needed within the scope and purpose of the job.
Service Areas:
* Children Looked After - Acting as a responsible parent and providing care and support.
* Children with Disabilities - Offering specialized support to children with disabilities.
* Children in Need - Assisting families in difficult times and protecting children at risk.
* Youth Offending - Helping to prevent reoffending among children and young people.
