We are looking for a skilled and enthusiastic Safe Discharge Therapist (either Senior Occupational Therapist or Physiotherapist) to support people being discharged from hospital with either restarts of existing Care Packages in their own homes, or readmission into a Care Home.

Following a lengthy stay in an Acute Hospital, people have usually deconditioned over their stay and lost confidence. Care homes and Home Care providers are often cautious of taking on complex moving and handling, which can result in more people being cared for in bed unnecessarily. This can lead to further health issues, worsening of mobility, falls and potential contractures. Through the work of the Safe Discharge Therapist these risks will be greatly reduced, and care staff will be empowered to positive risk take, enabling the individual to reach their maximum potential and well-being.

This therapist will support these people and ensure the Care Home or Home Care providers follow any manual handling plans sent home with them The safe discharge therapist will:-

Complete face to face visits to ensure a safe transition home and prevent readmission.

Oversee any adjustment to existing packages of care and ensure any onward referrals are completed.

Link with other services such as community physiotherapy team or Early Intervention and Prevention Team if ongoing support has been identified.

*The working hours and Pay*
* Working week will be a total of 37 hours
* Working hours are 09:00-17:00
* Pay £28.00 per hour (inclusive of holiday pay.)

*Essential Requirements*
* You need to have a DBS for this position.
* 2 Years + experience required.
