The post holder will seek to reduce and prevent the need for people to turn to statutory Mental Health Services through empowerment to address their social care needs in the community by delivering person centred one to one support and meaningful group activities across four locations.


The post holder will be supporting those who access the service through the development of a person-centred recovery model and a structured action plan, provide the clients with opportunities to engage in meaningful group activities, and support the development of their living skills.

This will be achieved by utilising the Adferiad Recovery Programme to support a person to develop an ability to self-assess their current circumstances, identify their goals for recovery and help address their social care needs.


Responsibilities:

Complete relevant intake forms, assessments and recovery plans for each individual referred to the service.


Develop successful engagement with services users based on foundations of trust and respect within a relationship built on clear professional boundaries.

Maintain accurate and up-to-date administration and caseload records, including accurate reports of all client interactions and discussions.

Supporting service users within the framework of the Adferiad Recovery Plan to address identified needs in the following areas:

  • feeling secure at home
- increasing independent living skills
- personal finance, budgeting, and accessing benefits
- mental and physical healthcare
- access to and managing of individual's property
- building and maintaining relationships with families, friends, professionals
- developing skills and interests
- pursuing training/educational/employment goals and aspirations
- pursuing leisure/recreational goals and interests
- developing links with the local community-based resources and opportunities
- recognition of and coping strategies in respect of risk, triggers and avoiding relapse

Signposting, joint work and referral on to other specialist agencies, intervening in crisis situation


To enable clients to exercise informed choices about their lives where possible and to enhance their social skills and participate in the community.


Person Specification:

  • Must be evidence of commitment to personal and career development relevant to the post.
  • Must have experience of providing support to individuals with serious mental illness.
  • Must have experience of developing and maintaining productive relationships.
  • Must have experience of supporting the target service user audience to engage in a variety of educational, community and social activities.
  • Experience of working with political sensitivity and of building and maintaining effective partnership and stakeholder relationships.
  • Experience of working with a wide range of partner agencies.
  • Must have a full UK driver's licence and access to a vehicle for work purposes.
