An opportunity has arisen for an experienced Band 6 nurse to join the South West Alliance (SWA) Local Access Point. We are looking for a dynamic, highly experience Nurse to join our team who are currently based within the locality of Stafford, Cannock and Seisdon. The Triage Nurse will take all new referrals for the local Community Nursing Teams in South West Alliance locality from all health care professionals, surrounding hospitals and G.P. practices and obtain all relevant information and documentation relevant to the referral.

The post holder will require excellent interpersonal skills to be able to manage all referrals, and forward them to the correct team to ensure a seamless process for all of the referrals. IT experience would be valuable but In house training will be provided during induction on MPFT systems.

  • The Triage Nurse will provide triage assessment, clinical support, assistance and professional expertise to all health care professional referrers requiring assistance and support with Admission Avoidance and hospital discharges.
  • The Service will be the access point for all community nursing referrals for the Locality referrals professionals requiring Health or Social Care Services.
  • The triage nurse will provide clinical triage and support where appropriate. This may range from signposting the referral caller to the appropriate service, making a direct referral to another service or offering immediate clinical advice or treatment.
  • The post holder is expected to represent the organisation in a professional and courteous manner at all times and provide advice and information using a level of clinical understanding and professional judgement.
  • To establish and maintain excellent communication links with all Health and Social Care Professionals.
  • To work independently, exercising assessment and evaluation skills to take a clear and comprehensive referral, for the community nursing teams in the locality obtaining all relevant documentation as appropriate for referral.

To provide high quality, clinical triage using knowledge, skills and critical thinking and professional judgement

To control and manage calls effectively by using skilled questioning, active listening and excellent verbal communication skills in a calm, professional manner

Identify and report any case of suspected or confirmed abuse to an individual, in accordance with agreed guidance and procedure.

Work as a member of the nursing team in conjunction with other professionals to promote seamless patient care.

Be personally accountable for professional and ethical actions and omissions in compliance with the NMC Code of Conduct.

Be prepared to work flexibly, in the interests of self-development and exigencies of the service. To actively participate in the on-going development of the role and support the development of the protocols and processes within the service.

Establish and maintain effective relationships and communications within the Primary Healthcare Team and also with other professional bodies, statutory, voluntary and private services, in order to enhance seamless patient care delivery.

Regularly participate in collection of audit data based around own activity and relating to patient care e.g. product, equipment and clinic audit, and team activity and organisation e.g. dependency audit.

Actively promote the correct implementation and upholding of relevant statutory measures and governmental directives.

Undertake personal education and training specific to role as identified in conjunction with team leader in order to carry out full range of duties.

Attend continuing education and in-service training to maintain an up to date level of professional knowledge and skill. This to include mandatory training sessions.

To undertake and participate in additional training requirements to enable the post holder to carry out the responsibilities of the role fully.

Contribute to the induction experience of new staff by demonstrating role, effecting introductions in the local organisation, and providing orientation information.
