Therapy Support Worker

The role requires day-to-day delivery of Therapy in the Community, working alongside the Physiotherapists, Occupational Therapists and Advanced Therapy Support Workers in patients own homes and care homes.
You will be delivering rehabilitation plans established by the team to work on achieving the goals set.This will require you to work within a large Integrated Community Team consisting of Community Nursing and a Frailty Team.
Our Core Therapy service operates a shift pattern between 08:30am and 16.30pm Monday to Friday and a shift pattern of between 10:30am and 18:30pm, at weekends and Bank Holidays.
The later shift pattern is covered by the Core Therapy Team and the Rapid Therapy Team on a rota basis.
You will be required to drive to your visits and be able to operate a laptop and write notes onto Systmone.
We hold weekly Therapy team huddles to discuss discharge planning of patients on our active caseload of which you will play an essential role.
