Our daily duties are rarely the same due to the broad and varied requirements of role. This allows our team to observe and support patient care at all points of their pathway.

The role generally provides the freedom to act creatively to perform/support with whatever tasks are required to expedite discharge and minimise re-admission likelihood.


We work collaboratively with our inpatient colleagues in numerous hospitals across the region and also alongside our colleagues in community mental health services.

Assertive Transitions Service exists to motivate patients across secure hospitals in the East Midlands to work towards discharge into the community and work to put practical measures in place to make this a reality, sooner.

We then continue to support individuals in the community for up to 12 months post discharge, working to minimise the likelihood of re-admission.


All of this work is done alongside existing RC/CCO to ensure all our patients receive a high standard of wrap-around, holistic care.


We operate a very supportive atmosphere whereby we all work collaboratively towards a shared goal and support each other with our caseloads as much as possible.

We also have a robust system of supervision to ensure that everyone in the team feels secure in their work.


We have a huge variety of opportunities in our expansive Trust covering geographically from South Yorkshire, Nottinghamshire to Leicester, providing mental health inpatient and community services, community health services, forensic mental health services and secure prison healthcare across the East Midlands.
