The Community Rehabilitation Team is a recently formed service which offers a specialist rehabilitation focused approach to support SWLSTG service users in a variety of settings including out of area and local inpatient rehabilitation units, supported rehabilitation placements and community teams.

The service has a five Rehabilitation and Placement Practitioners, in addition to a Consultant Psychiatrist (3 days per week), Social Worker, Occupational Therapist, Psychologist and Community Psychiatric Nurse. These jobs will be to ensure that a community rehabilitation approach is maximised to support trust service users on the rehabilitation pathway and are effectively supported in the least restrictive setting both inpatient and community. A vital component of these exciting roles are the interface and support to the South London Partnership (SLP) Complex Care Programme. They will aim to support the SLP Complex Care programme in several different areas and work closely to consider the care management of a complex client cohort in inpatient and community settings. We are seeking enthusiast people who are registered with either the NMC, HCPC or Social Work England, have completed their Preceptorships or Assessed Year of Supported Employment (AYSE) and have experience of working with adults with severe and enduring mental health needs, ideally with an understanding of how placements in rehabilitation wards or supported accommodation do fit within the Recovery model and their role in enabling clients develop greater independence. Main duties of the job
  • Work closely with the two SWLSTG inpatient rehabilitation units, Phoenix and Burntwood Villas, as well as inpatient rehabilitation units in the South London Partnership (SLP) footprint and out of area.
  • Work closely with housing providers and other agencies to ensure appropriate care plans are in place promoting independence, recovery, and rehabilitation.
  • Lead on caseload management of clients in inpatient rehabilitation units.
  • Facilitate client step downs from hospital to supported accommodation placements in collaboration with social care.
  • Contribute to more effective systems for future planning and reviewing of clients in rehabilitation placements.
  • Ensure high quality care is evident in referrals to the SLP Single Point of Access (SPA).
  • Work with the SLP Clinical Assessment Team.
  • Support existing providers such as Riverside and Bridge.
  • Support the flow of care throughout the inpatient and community pathways and reduce delays in discharge.
  • Maximise community stakeholders, including the voluntary sector, to enable full recovery and independence for our clients.
  • Provide support and guidance to community teams, supported housing providers and key partner agencies.

Maintain a high standard of individualised client care, recognising factors and events that are likely to affect the mental, emotional and physical well-being of clients/carers and provide advice and support when required. Promote social inclusion and physical and mental well-being using evidence-based practice. This is underpinned by the principles of hope and recovery, health promotion and relapse prevention. Assess clients needs and strengths through a collaborative approach, ensuring that all clients have care plans and that these are implemented and regularly reviewed/updated. Communicate effectively with relevant agencies to ensure that an integrated programme of care is provided throughout. Develop and maintain effective working relationships with inpatient facilities regarding admission and discharge of clients. Participate in partnership working with other care coordinators in the admission, discharge and transfer of clients in accordance with Trust Policies. Operate as a resource of specialist knowledge and expertise for community support providers (Supporting People providers, Housing Associations, Landlords) with the aim of reducing re-admissions and clients moving back to higher levels of support. Facilitate assessment by potential move-on accommodation providers at an appropriate time. Liaise with all parties and support clients practically and emotionally through the assessment and move-on process, including visits, transitional leave and final move. Provide clients and carers with verbal and written up to date information on the services offered by the Trust, how to express their views, complaints procedure. Raise safeguarding concerns for clients in placements when necessary, monitoring the progress of any Enquiry and assisting with investigation and implementation of any subsequent Safety Plans. Monitor the therapeutic effects and side-effects of prescribed medication providing the client (and others as required) with advice and information. Develop and maintain a high standard of practice, based on assessment, using a range of psychosocial approaches, within a health and psycho-social framework. Refer carers for their own needs assessment to monitor efficacy. Liaise with both the Local Authority and SLP / ICB for jointly funded placements. Attend the Accommodation Panel as needed. Person Specification Education and Qualification Essential
  • RMN (current NMC registration) or Occupational Therapist (current HCPC registration) or Social work registration (Social Work England)
  • Evidence of continuing Professional Development
  • Relevant Degree or Diploma
