We are seeking a highly motivated and compassionate Rehabilitation Assistant to join our Community Neurorehabilitation service at Guy's and St Thomas' NHS Foundation Trust.

About the Role

This is an exciting opportunity to work in a dynamic and creative service, supporting adults with neurological diagnoses and progressive conditions. As a Rehabilitation Assistant, you will work closely with our multidisciplinary team, including Physiotherapists, Occupational Therapists, and Speech and Language Therapists, to provide high-quality rehabilitation to our patients.

Key Responsibilities
  1. Manage patient caseloads and allocate referrals in collaboration with the team.
  2. Organize working schedules and meet clinical contact targets.
  3. Work in various community settings, including neurorehabilitation service bases, out-patient clinics, and patient homes.
  4. Participate in decision-making regarding patient rehabilitation programs.
  5. Complete risk assessments and evaluate risk in relation to patient and staff safety.
  6. Treat patients according to MDT-developed programs, monitoring progress and adapting treatment as needed.
  7. Maintain comprehensive patient records and complete administrative tasks.
  8. Communicate discharge plans and health needs with patients and relevant others.
  9. Report concerns about patient welfare directly to treating therapists and supervisor/line manager.
  10. Assist with student development on placement.
Requirements

To be successful in this role, you will need to possess excellent communication and interpersonal skills, with the ability to work effectively in a team environment. You will also need to be flexible and able to adapt to changing priorities and circumstances.
