and main responsibilities
• To provide specialist, evidence based Occupational Therapy interventions to a caseload of inpatient hospice and community patients to ensure optimal clinical outcomes and quality of life. To work with patients to assess their needs, using specialist assessment processes and work collaboratively with the patient, their family, carers and the MDT to set patient identified goals which incorporate 'what matters' and are meaningful to them.
• To evaluate treatment outcomes and maintain patient records (paper and electronic).
• Contribute to the improvement and development of Occupational Therapy services.
• To provide evidence of compliance with clinical governance standards in order to contribute to the Trust's assurance for CQC quality and safety outcomes
• Contribute to meeting the training needs of Occupational Therapy staff, other professionals and students in areas of skill and knowledge relevant to the post holder's expertise.
• To undertake quality improvement projects as appropriate to meet the needs of the service.
• To complete risk assessments to ensure that risk in the service is managed and reported as required. All Occupational Therapy staff are responsible for the safe use of equipment and person handling including ensuring the competence of staff, carers and patients.
• All Occupational Therapy staff are required to participate in regular supervision sessions as agreed for their individual needs in line with the occupational therapy service supervision process. This includes clinical and management supervision, as well as appraisal and reviews.

Essential criteria
Degree / Diploma in Occupational Therapy HCPC registrationDesirable criteria
Member of the royal college of occupational therapy Completed post graduate communication training postgraduate experience in palliative careExperience

two years post graduate OT experience in a physical setting within the NHS Experience of working with patients with a palliative diagnosis Band 5 experience of core rotational roles Experience of delivering education including IST programmes Experience of working with complex multi pathologies, co morbidities and long term conditionsDesirable criteria
Experience working with neurodegenerative conditions e.g. MND Experience delivering supervision / teaching / mentoring to junior staff or students Experience of working in the community setting 3 years post graduate experience working in an NHS clinical settingTechnical skills & competencies
