Job summary

We have a new and exciting opportunityto work with ourteam as a Perinatal Nurse. Main duties of the job

To work as part of a multi-disciplinary team delivering care to babies within the home and hospice environment, respectingthe skills, expertise and contributions of others in the provision of holisticcare and support to babies and their families using Derian House services.

To be responsible for the assessment of the babys care needs, includingthe development, implementation and evaluation of programmes of care.

To assist in the management and organisation of work as required by the ServiceLead, ensuring effective communication is maintained at all times and toprovide support for team members.

To be responsible for making initial contact with families who have beenreferred in to the service whether that be within a hospital or homeenvironment ensuring correct services are offered when appropriate.

Be an advocate for families in the early stages of diagnosisante-natally and post-natally, initiating advanced care planning and ensuringthey transition effectively into relevant services.

Derian House is rated Outstanding by the CQC; Derian House provides symptom management, palliative and end-of-life care, as well as short break respite care to babies, children, young people and their families with life limiting conditions across Lancashire, South Cumbria and Greater Manchester. Supported by our dedicated multi-disciplinary teams we are able to provide a wide range of support and care in a variety of settings, including in-house in our Hospice in Chorley and in the wider community.

We actively encourage ongoing professional development and have an in-house clinical education team working within a dedicated clinical skills lab to enhance and improve training and competencies.

Job responsibilities

To act as a positive role model byusing evidence/research based care and practice.

To be proactive in identifying ways ofimproving practice. Contributingpositively to the development and audit of standards.

Provide and oversee the delivery of high quality, clinicallyeffective and evidenced based nursing care and practice in accordance withDerian House policies, standard operating procedures and guidelines.

Ensure that care for babies is assessed, planned,delivered and evaluated respecting their daily routines as far as is possible.

To be responsible for providing safe and effectivecare in line with the hospices operating procedures and guidelines and act asa resource for all members of the multi-disciplinary team providing specialist professional,care and support.

To adapt the delivery of care accordingto the emotional, physical and environmental demands.

To participate in the primaryassessment of newly referred babies.

To refer to the Service Lead for advice/guidanceduring a shift as and when required.

Ensure the respect, dignity and rights of babiestheir families are upheld at all times.

To be competent in Derian Houses coreand clinical competencies and individual care needs.

To be responsible and accountable forthe safe administration of medicines within the home ensuring medicinemanagement policies and procedures are adhered to as per NMC guidelines.

To attend ward rounds in a wide varietyof settings to ensure that referrals are received and actioned effectively.

To work alongside the clinical teamwithin Derian House when requested by Service lead or shift leader insupporting staff with palliative and end of life care for babies.

Person Specification

Qualifications

Essential

RSCN, RN (Child), RGN & RNLD with a proven track record of neonatal competency. Minimum of three years post- registration experience.

Desirable

Palliative care qualification. Willingness to study to a minimum of degree level
