Job summary

Providing Clinical Expertise and visible leadership and support for the home haemodialysis (HHD) programme. Enabling staff to ensure that the fundamentals of care are correctly and actively in place and that highest standards of holistic care are achieved in the delivery of care, training and treatment. To directly manage the HHD service and the nursing staff. The expectations of this role will be met by professional leadership, skilled and expert practice and management, direct clinical care, education and training. Develop and update guidelines and related practice in line with current research

Monitor standards of care; manage selection, deployment, training and development of staff and students. Assesses and develops and implements specialised nursing packages of HHD care and is responsible for maintenance of standards by all members of the multi-disciplinary team.

To create and support an environment of lifelong learning and continuous professional development. Provides highly specialised nursing advice to ensure appropriate treatment and care is delivered to the patient at all times. Refers and orders specialised tests in line with agreed protocols and clinical governance guidelines To assist in the development of evidence based practice in the renal service.

Main duties of the job

Ensure that all care delivered is evidence based and that guidelines are up to date.

Promote and provide a comprehensive training package for patients selecting HHD.

Liaise with the multidisciplinary team to ensure that HHD is promoted & patient information needs are met in full.

To ensure that the Patients know best (PKB) is an evidence based and current source of information for patients selecting HHD.

Selection, deployment, training and development of staff and students

To create and support an environment of lifelong learning and continuous

professional development

Refers and orders specialised tests in line with agreed protocols and clinical governance guidelines.

To assist in the development of evidence based practice in the renal service.

Educational strategies that ensure patient care needs are met.

Provides highly specialised nursing advice to ensure appropriate treatment and care is delivered to

the patient at all times in line with NICE guidelines and UK Kidney association guidelines.

Is accountable for the provision, organisation and direction of the nursing care for the patient

in his/her care.

Attends meetings & groups as required by the renal service eg Clinical practice or governance groups
