We are looking for a passionate and highly motivated specialist occupational therapist or physiotherapist or Registered mental health nurse to join our Bradford District Care NHS Foundation Trust Older Peoples Mental Health Service (OPMH) in a new Outreach Team which has been established to support residents in care homes (and those living in the community if and when needed) who require enhanced support and/or present in crisis out of the usual hours of our existing Older People's Mental Health (OPMH) service.


You will be joining a dynamic team which is structured as a B7 Gate Keeper RMN, B6 RMN, this post, a B4 therapy assistant practitioner and 2 x B3 HSCWs.

The Outreach Team, hosted within the BDCFT Dementia Assessment Uni, will work closely with care home providers across the B&C district who routine provide care for people living with dementia (or suspected dementia).

You will work within the OPCMHT across the Bradford and Craven district.

You will be working with the care home staff, our health and care professionals, and the individual and their family with the goal of supporting the individual to live well for longer in their usual residence with quality of life; avoiding the need for unnecessary transitions of care (e.g. to A&E/acute hospital admission) and/or heightened levels of formal care and support.


The Outreach Team will receive referrals from the Care Home Liaison team, other older people mental health professionals and potentially care homes directly - for individuals requiring additional support to prevent escalation of distress behaviours and where there is a need to support care home staff to continue to provide personalised care and support aligned to evidence based models of care, for example CLEAR Framework and the NHSE/I Dementia Well Pathway.


We seek an enthusiastic AHP or RMN to join this new and evolving Outreach Team in recognition of the unique contribution of these professions in delivering holistic person-centred assessment and care and support for people living with dementia.

AHP's and RMN's have a unique role in supporting individuals with dementia to participate meaningfully in daily life, to be empowered to self-care and to use strategies (with the support of others around them) to avoid crisis and escalating distress behaviours through holistic assessment and care and support planning.
