Physical Health Liaison Worker

£25,707 per annum

Surrey

People living with severe mental illness (SMI) face one of the greatest health inequality gaps in England.

People with an SMI are three times more likely to attend Accident & Emergency (A&E) with an urgent physical health need; almost five times more likely to be admitted to A&E as an emergency; twice at risk of obesity and diabetes; three times at risk of hypertension and metabolic syndrome; five times at the risk for dyslipidaemia than the general population.

This new pilot initiative aims to reduce this inequality along with the pressures on primary care.

The aim is to increase the quantity and quality of physical health checks carried out for patients by coordinating and facilitating physical health checks for people living with severe mental illness in Surrey.


Community Connections Surrey will support delivery of enhanced physical health checks in the community by working with the individual to help facilitate their attendance and participation in regular physical health checks with their health care professionals.

This is a fixed term post that ends on 27th January 2024

This is a full time role requiring the post holder to work 37.5 hours per week.
