The Carers Leads' primary role is to support unpaid carers and the patient from the time the person is admitted to hospital to the point of discharge.

The Carers Lead will assess the needs and complete a support plan for the unpaid carer ensuring the patient has the required support needed to be discharged from hospital thus lowering the risk of readmission to hospital.

The carer should be in a better position to continue with their caring role once the patient has been discharged, and is better able to access continuing support.


  • Primary_
  • Providing support to unpaid carers who have a loved one in hospital.
  • Assessing carers needs prior to discharge, at Multidisciplinary meetings & on the point of discharge.
  • Ensuring care packages for patient and any other services are in place before discharge. Communicate options for support on discharge and how to access support.
  • Advocacy support: Advocacy and facilitating communication between all members of staff including medical, management, discharge coordinators, discharge teams, social services, pharmacists and community support groups
  • Providing information and advice.
  • Main Duties:_
Identify unpaid carers involved with patients in NWP who are residents in LB of Harrow. Register these persons with Harrow Carers so that they may continue to receive support following discharge

Engage carers prior to discharge and carry out an assessment of needs.

Ensure carers are aware of hospital policies for the inpatient, engaging with social services, understanding carers rights and ensuring they are aware of all aspects of the discharge process and how to access information

Complete support plans for patients and their carers

Ensure all unpaid carers are provided with a carers card. Signpost to carer peer support groups and medically specific support groups so carers can support each other.


Refer to counselling services and wellbeing programmes to reduce isolation and maintain and improve the health and wellbeing of unpaid carers.


  • Maintain a regular presence at the hospital, working closely with hospital staff to aid discharge from A&E and wards. Ensure staff at all levels are aware of the importance of carers in the hospital and in the community and how they may benefit the work of staff, treatment, recovery and discharge
  • To record accurate and updated client information in line with Harrow Carers confidentiality policy and within relevant databases allowing a comprehensive assessment of the patient's needs and priorities.
  • Provide support, confidence and advocacy for patients and their carers; facilitating them to access information and advice regarding services to meet their needs during the transition back into the community
  • To assist patients and their carers to participate in best interest meetings, treatment plans and discharge meetings and advocate on their behalf with the view to providing them with a care package that adequately addresses their needs in the community.
  • To support unpaid carers to understand their rights and entitlements whilst in hospital including the right to complain.
  • Signpost and refer unpaid carers for specialist and/or ongoing support within the community through other organisations
  • To promote means by which a carer can better manage their own health by planning and facilitating regular workshops on Health Management i.e. Manual Handling and First Aid for older patients
  • Ensure that the project is publicised widely within the hospital/setting using appropriately developed publicity materials
  • Raising awareness of unpaid carers within the hospital setting, promoting their interests to hospital staff and social workers professionals
  • Working closely with the Hospital Discharge Manager to coordinate services where possible, identifying areas for improvement and develop means to address these
  • To work independently and to manage own time effectively and make the best possible use of resources available
  • Carry out all necessary administration for the project. This will include working with the Project Manager to develop and maintain appropriate monitoring systems, obtaining regular feedback, preparing statistical and other reports

  • To actively participate in regular supervision meetings, providing monitoring reports and feedback on project progress and needs
  • To attend and represent the views of unpaid carers at relevant meetings and appropriate Steering Groups/Networks; advising and reporting on the development of this role and the impact upon the discharge process
  • Ensure that the needs of older patients; from all ethnic groups within Harrow are fully catered for. This will include organising an interpreting service where necessary.
  • Raising awareness of and promoting the work of Harrow Carers within the hospital and community
