We are looking to recruit an enthusiastic and highly motivated Band 3 Rehabilitation Assistant to our community team. The role involves working with patients in their own home to provide rehabilitation services and promote independence.

Successful applicants will be part of an enthusiastic, patient focused, holistic team caring for patients in their own homes. Our Multi-disciplinary team comprises of Physiotherapists, Occupational Therapists, Nurses, Healthcare and therapy assistants, as well as support from administrators.

We work collaboratively with GPs, acute hospitals, Social care, Hospices and other third sector organisations.

You will have the opportunity to work with patients with a wide range of clinical conditions.

We have a well-established structure for clinical supervision and opportunities for practice development including in-service training. This will provide you with fantastic opportunities to enhance your existing knowledge and skills.

  • To be an active member of the clinical team working within standard operating procedures and pathways to manage your own workload independently but with reference to a supervisor within the clinical team.
  • To support the team by assessing patients and implementing programmes of care ensuring that you follow professional standards, promote best practice and work within your sphere of competence and knowledge.
  • To undertake all aspects of clinical duties as an autonomous practitioner including assessing, treating and managing own caseload of patients and maintaining professional documentation.
  • Build and maintain relationships with other local services to facilitate appropriate referrals and high quality healthcare.
  • To deliver appropriate treatment in a variety of settings including the patient's own home, residential and nursing homes and day centres.
  • Actively participate in Continuing Professional Development to support your own development and the development of the service. Including participating in any audit or research being completed by the service.
