Job summary

The post holder will be responsible for line managing a Continuing Healthcare (CHC) clinical team.Supporting the CHC Review and Assessments Manager and taking responsibility for the management of the day to day work assigned to the clinical service line. Ensuring that the clinical team, when carrying out their duties, work in accordance with the latest statutory requirements, in particular The National Framework for NHS Continuing Healthcare and Funded Nursing Care and that the practice is in line with the Good Practice Guidance revised 2012 and within the Code of Professional conduct NMC April Manage the Team's, including developing and managing the team's business plan, progress and reporting risk and issue Liaise with appropriate functions within the ICBo Case management across all age Continuing Healthcareo Take a lead in the coordination of training & development and recruitment activity across the The above is only an outline of the tasks, responsibilities and outcomes required of the role. The job holder will carry out any other duties as may reasonably be required by their line manager.

Support the team in meeting it's KPI targets

Oversee the investigating and responding to complaints from their CHC team.

Lead in supporting the CHC portfolio in reporting and responding to Safeguarding.

Main duties of the job

o Contribute to performance improvement, taking a lead for identified areas where agreed. Responsible for line management of the Continuing Healthcare (CHC) clinical team and providing Maintain and improve quality of assessments/ Decision Support Tool by clinical staff and maintain consistency of Ensure that the CHC team use national and local assessment tools appropriately and adhere to the lead times. Education / Qualifications

Essential

Registered Nurse with the Nursing and Midwifery Council (NMC) or other recognised approved body, for example, HCPC, Social Care England and/or educated to degree level in relevant subject or equivalent level qualification. Post-graduate degree or equivalent Knowledge of project principles, techniques and tools, sucha as PRINCE 2 Foundation and MS Project

Experience

Demonstrated knowledge and understanding regarding the application of NHS Continuing Healthcare, NHS Funded Nursing Care, Complex Packages of Care, Appeals and Disputes Significant experience of successfully operating in a politically sensitive environment Evidence of continued professional development Member of relevant professional body Experience of co-ordinating projects in complex and challenging environments Experience of managing risks and reporting
