Hospital Team Social Worker

Edenbrown Synergy is currently recruiting for an adults Hospital Social Worker for a Hospital team with the London Borough of Merton.
The Role:
- Experience in Hospital discharge
- Experienced in working towards meeting short deadlines
- With experience to timely undertake joint assessments with our health partners
- With experience and with excellent understanding of the CHC framework
- With very good English written grammar and ability to write comprehensive, strength based Care Act assessments, Risk assessment safeguarding enquiries and other professional reports
- With experience in the role of Safeguarding Enquiry Officer
- With excellent customer service manner, able to have comprehensive, clear conversations with diverse customer/ carers/ family groups and abilities
- Assertive and not afraid to hold difficult conversations
- To have an excellent time keeping
- To be able to manage their workload and be fluent in using various IT tools in assistance of the workload management
- To have an IT experience and is able to use Microsoft teams, Zoom as assistive communication/ assessment tools
- Is driver and preferably live within short drive of Lb Merton
- Is an excellent team player and able to recognise professional boundaries and responsibilities recognise professional limits and seeks timely supervision
Essential Requirements:
- Social Work Qualification
- Experience as a social worker within adult social care
- Social Work England Registration
