Snapshot:
Role: Healthcare Assessor
Hours: Full-time, 35+ hours per week, with additional on-call hours on a rota basis
Location: Newcastle
Salary: Negotiable based on experience

As part of our team, you will need a full valid UK driving license with access to your own vehicle due to the nature of our business and role requirements.

Role Overview:

  • Developing care plans and risk assessments
  • Overseeing the safe administration of medicines
  • Conducting observations, spot checks, and Quality Assurance
  • Fostering a culture of person-centered care within the community and at Routes Healthcare
  • Maintaining high standards of record-keeping in line with regulations and enabling effective communication with professionals
  • Preparing case studies to share with relevant parties when necessary

Experience and Qualifications:
- Minimum of 1 year of experience in community care
- Sound understanding of the health and social care sector and person-centered care
- Experience in leading shifts and providing high-quality care
- Familiarity with working with palliative care clients
