To establish and provide a diabetes nutrition support service to patients with diabetes, independently and in partnership with Diabetes Specialist Nurses. You will manage a diabetes caseload with the Community Diabetes Nursing Service with varying degrees of nutritional complexity without direct supervision and exercising a large degree of autonomy.

You will be responsible for using current evidence and knowledge to propose and evaluate alternative approaches to dietetic practice in community diabetes, aiming to improve outcomes within available resources. This will support government policy by developing new ways of working.

You will be an integral member of the Community Diabetes Service and take a major role in the delivery of diabetes structured education programmes X-PERT Diabetes Prevention and Management and DAFNE. Act as a Specialist Dietetic Lead for adult Type 1 and Type 2 diabetes and the education of adults with diabetes.

Main duties of the job
  • Delivery of a comprehensive, skilled, and evidence-based Diabetes service to meet the needs of the population.
  • Providing leadership skills, management, and development within the diabetes team.
  • Maintain a clinical caseload of complex high-risk patients.
  • Continuing responsibility for assessment of health needs and development and evaluation of standards of care.
  • To participate in audit/evaluation for adult community diabetes and other projects as required by the X-PERT and DAFNE Programmes, Community Health Trust, and Community Diabetes Service Manager.
  • To provide education and training to the wider multidisciplinary team in matters concerning the nutritional and lifestyle management of patients with diabetes including those with complex presentation.

  • Dietician registered with Health Professions Council
  • Teaching Certificate or equivalent experience
  • Experience and training in the field of diabetes to master's level or equivalent e.g., BDA Advanced Dietetics in Diabetes course, EXTOD
  • BSc in Nutrition & Dietetics
  • Highly developed specialist Diabetes knowledge achieved through attendance of post-graduate degree level course e.g., Warwick course.
  • Evidence of continual professional development and education
Experience Essential
  • Post registration experience in diabetes, cardiovascular disease, and obesity.
  • Experience of managing a range of complex patients with Type 1 and Type 2 diabetes (e.g., TBI, CF, low BMI, patients with CGM, those requiring nutritional support).
  • Experience of provision of training/education to range of staff and patients including the management of complex patients
  • Ability to effect and manage change
  • Advanced communication, negotiation, decision making and organisational skills
Knowledge Essential
  • Diabetes specialist Counselling course (e.g., Knuston Hall) or equivalent)
  • Able to utilise research and audit to maintain and promote best practice.
  • Ability to apply nutritional knowledge in a variety of settings.

  • Ability to meet physical demands of the post including manual handling of clinical equipment.

  • Teaching & Presentation skills
