The Clinical Support Practitioner will be responsible for constructing individual care plans and conducting telephone assessments along with attending home visits.


In return, the Clinical Support Practitioner will expect a salary of up to £26,000, 26 days paid annual leave, pension contribution, access to thousands of perks and discounts, optional health and wellbeing cover and life assurance at 4 x your annual salary.


Role & Responsibilities:

  • Attend home visits (when appropriate)
  • Conduct telephone assessments and construct individual care plans
  • Advise on 'reasonable adjustments' that can be made
  • Work with team members to ensure the customer is fully supported

Skills & Requirements:

  • Previous experience of a clinical background
  • Excellent customer service experience
  • Previous experience of constructing care plans
  • Strong organization skills and high level of attention to detail
