This position is designed to contribute to avoiding hospital admission, facilitating early discharge from hospital and maintaining patients in their own homes.
This post offers the opportunity to work closely with all members of the MDT both in primary, secondary care, Adult Services, as well as the community and voluntary sector.
The role is about coordinating care or support across organisations rather than referring on, following up with patients to see how they are getting on and lastly, it's about bringing medical and non-medical professions together under one banner to provide proactive holistic care.
