Working at St Mary's Hospital with the Gynaecology team to support Cancer Clinical Teams providing personalised care for people living with and beyond cancer.

Please note this is very much a clinical role working directly with patients and or cancer professionals. This post is predominantly patient facing.

The role requires a team player but also someone who can work independently and use their initiative - it is an excellent opportunity for someone who is enthusiastic with excellent organisational, IT and interpersonal skills and who is passionate about ensuring the best possible compassionate care for people living with and affected by cancer.

Relevant health and social care experience at AfC Band 3 or equivalent required.

Main duties:

Improving the care we deliver for people living with or affected by cancer is a key priority for us.

This post is full time, permanent contract.

The Macmillan Cancer Care Support Co-ordinator role is an integral part of the Cancer Workforce and involves direct contact with people living with and beyond cancer, to support them throughout their cancer care.

You will work as part of different clinical cancer teams and will be a point of contact for patients and carers, supporting and co-ordinating their aftercare; including assisting with personalised care and support planning (based on holistic needs assessment), treatment summaries, providing health and well-being information and support, supporting personalised stratified follow up and helping to promote cancer awareness.

You will work closely with all grades of medical and nursing staff and wider health care teams. This role involves competency based practice supported by an established learning and development programme.
