This is a great opportunity to join our Crisis Resolution Home Treatment Team as a Clinical Co-Ordinator.

The Crisis Care Centre, at Harplands Hospital, Stoke-on-Trent has been designed to the highest level of specification.

This modern, purpose designed unit, will be the central point for all those in need of a Crisis Assessment and Intervention, and it also provides the Place of Safety for those patients detained on a Sec 136 by Staffordshire Police requiring assessment.


The Crisis Care Centre is an essential component of the urgent care pathway and provides an all-age single point of access to mental health services in Stoke on Trent and North Staffordshire, whilst also offering a 24-hour, seven-day crisis service with the additional offer of intensive home treatment as an alternative to an acute inpatient admission.


The Crisis Care Centre, in line with the NHS Long Term Plan will be expanding this offer too support individuals in crisis by receiving referrals via NHS 111, in addition to the dedicated line for ambulance, police, GP's and other professionals requiring an urgent mental health response.


The post holder will be expected to provide strong clinical leadership as well as work as a member of the multi-disciplinary team within the Crisis Care Centre co-ordinating a fast-paced dynamic service on a shift to shift basis over a 7 day 24 hour period.


The post holder will ensure that all referrals received into the service are triaged and assessed according to risk and needs, ensuring that appropriate advice and intervention is offered.

The post holder will require relevant links and understanding of wider mental health and discharge pathways.

Clinical responsibility for the quality and performance of services offered at the Crisis Care Centre, in conjunction with the Operational Team Manager, ensuring processes are in place for the delivery of safe and compassionate care in accordance with Trust policies, procedures and values.


Lead a multi-disciplinary team of staff in the delivery of high-quality evidence-based practice, ensuring that regular supervision and appraisal takes place.


The post holder will be responsible for the assessment of care needs, the development, implementation and evaluation of recovery-based care programmes and the setting of standards of care to patients and their families.


To ensure that practice remains abreast of new developments and to develop and maintain standards and implement change in respect of this.


Ensure systems are in place to manage administrative and operational resources in a cost-effective manner, evaluating the standard of service delivered.


Co-ordinate multidisciplinary working, maintaining excellent communication and working relationships with all team members, to ensure effective interface and continuity of care between disciplines and services.


Role model best practice, where appropriate undertaking clinical procedures to a standard that ensures safe and effective care and complies with the relevant Code of Conduct or relevant professional bodies.


Management of clinical care at the Crisis Care Centre, ensuring that clinical pathways and Trust values underpin the practice and delivery of care.
