Job summary

We are a high achieving, friendly training and research practice currently looking to recruit a forward-thinking and committed Advanced Nurse Practitioner to join our growing multi-professional team. We offer a supportive environment and have an excellent reputation with Dudley. Ideally the hours for this post are Tuesday mornings from 7am and all day Thursday and Friday. We require 25 hours but would consider offering additional hours for the right candidate.

Main duties of the job

Thesuccessful candidate will:

have a current UKprofessional registration;

be a qualified prescriber;

work as anautonomous practitioner, working independently within their professionalboundaries and in conjunction with other allied health professionals to assess,diagnose and manage conditions of patients attending within primary care;

be able to demonstratesafe and competent clinical decision-making, critical thinking and expert care;

initiate referrals ifappropriate, offer emergency appointments and home visits;

have a flexible approachto working hours as fits in with the practice needs;

be caring andcompassionate;

have excellentcommunication skills to meet the needs of patients and to communicateeffectively as part of the clinical team;

abide by our policies andprocedures;

be able to provide a nurse management role

Our continually growing list size is currently just over 11600 patients and comprises 6 GPs, an ANP, a Specialist Nurse Practitioner and a team of Practice nurses, as well as a full complement of support staff.

Job responsibilities

JOB SUMMARY

To workas an autonomous practitioner, responsible for the delivery of advanced nursepractitioner services, working as part of the practice multidisciplinary team, deliveringcare within their scope of practice to the entitled patient population.

PRIMARY RESPONSIBILITIES

Thefollowing are the core responsibilities of the advanced nurse practitioner or nurse practitioner.There may be on occasion, a requirement to carry out other tasks; this will bedependent upon factors such as workload and staffing levels:

a. Develop,implement and embed health promotion and well-being programmes

b. Managepatients presenting with a range of acute and chronic medical conditions,providing subject matter expert advise to patients

c. Implementand evaluate individual specialised treatment plans for chronic diseasepatients

d. Identify,manage and support patients as risk of developing long-term conditions,preventing adverse effects on the patients health

e. Provideadvanced, specialist nursing care to patients as required in accordance withclinical based evidence, NICE and the NSF

f. Undertakethe collection of pathological specimens

g. Requestpathology services as necessary

h. Processand interpret pathology and other test results as required

i. Maintainaccurate clinical records in conjunction with extant legislation

j. EnsureSNOWMED codes are used effectively

k. Prioritisehealth issues and intervene appropriately

l. Supportthe team in dealing with clinical emergencies

m. Recognise,assess and refer patients presenting with mental health needs

n. Extendedand supplementary prescriber, adhering to extant guidance

o. Supportpatients in the use of their prescribed medicines or over the counter medicines(within own scope of practice), reviewing annually as required

p. Contributeto practice targets (QOF etc.), complying with local and regional guidance

q. Liaisewith external services / agencies to ensure the patient is supportedappropriately (vulnerable patients etc.)

r. Delegateclinical responsibilities appropriately (ensuring safe practice and the task iswithin the scope of practice of the individual)

s. Supportthe clinical team with all safeguarding matters, in accordance with local andnational policies

t. Understandpractice and local policies for substance abuse and addictive behaviour, referringpatients appropriately

u. Deliveropportunistic health promotion where appropriate

v. Overallmanagement of the nursing team, providing guidance when necessary, acting as amentor to students and newly qualified staff

w. Maintainan effective nursing staff rota, ensuring all clinics are staffed appropriately

x. To have a lead role in regular nursing meetings and support thechanging roles of the nursing department and nurse development as requiredover time to fulfil the surgery's requirements

SECONDARY RESPONSIBILITIES

Inaddition to the primary responsibilities, the advanced nurse practitioner maybe requested to:

a. Actas the audit lead, effectively utilising the audit cycle

b. Actas the IPC lead for the practice, ensuring compliance, undertaking audit asnecessary

c. Monitorand ensure the safe storage, rotation and disposal of medicaments

d. Participatein local initiatives to enhance service delivery and patient care

e. Supportand participate in shared learning within the practice

f. Developsan area of specialist interest, taking the lead within the practice

g. Continually reviewclinical practices, responding to national policies and initiatives whereappropriate

h. Participate in thereview of significant and near-miss events applying a structured approach root cause analysis (RCA)

i. Drives the developmentof nursing services within the practice, liaising external agencies andprofessional organisations as required

J. Develops practice administrative and clinicalprotocols in line with the needs of the patient and current legislation
