About the Role:
In the Children in Care/Leaving Care Team, you will be a vital figure in the journey of children under long-term care, whether through Care Orders or Section 20 arrangements. Your duties will include advancing long-term care planning, facilitating pathway planning, and assisting young individuals as they transition into adulthood. By closely collaborating with families, foster carers, residential settings, and semi-independent establishments, you will provide essential support to address the diverse needs of the children.

- Leverage your expertise in statutory frameworks, policy, and best practices to ensure top-tier care and support.
- Exhibit proficient IT skills to efficiently handle caseloads and data management.

- Utilize excellent interpersonal skills to establish and maintain fruitful relationships with colleagues, partners, agencies, and most importantly, the young individuals and their families.
- Carry out assessments to identify children's needs and create suitable intervention strategies.
- Collaborate effectively with various agencies to guarantee holistic care and support for the children under your responsibility.
- Manage court proceedings under the Children Act 1989 & 2004 with precision and professionalism.
- Work towards achieving stability for children, ensuring they have nurturing and secure environments for their growth.
- Actively contribute to enhancing and progressing the Children in Care Team.

Person Specification:

Qualifications:
Qualified Social Worker with a recognized professional qualification (e.g., BA Social Work, MA Social Work, DipSW).
Current registration with Social Work England.
