An exciting opportunity has arisen for a Assistant Practitioner within the Woodbridge Integrated Neighbourhood Team (INT) on a permanent basis.


We are looking for a Assistant Practitioner who is passionate, committed, innovative, enthusiastic, and highly motivated to delivering high quality care within a community setting.

This role requires a working pattern of either Monday to Friday Or Tuesday - Friday Friday finishing at Weekend work will be on a rota and time off in the week given in lieu.

The Woodbridge INT is based in Melton and provides multidisciplinary care to the surrounding locality. The team is committed to holistic and patient centred care in the right place, at the right time. The team consists of Occupational Therapists, Physiotherapists, District & Community Nurses, Assistant Practitioners, Generic Workers, and Social Workers. The team is led by an INT Manager, supported by a Care Coordinator and Administrator.


Although we expect our staff to work autonomously, we're a very supportive team and work closely together to achieve the best outcomes for our patients.

Community experience would be an advantage, but the role includes a comprehensive trust induction followed by a local team induction.

We offer a variety of training to consolidate and maintain your clinical skills.

To manage a defined caseload of patients in the community under the guidance of registered Therapy clinicians.
To respond, with support from a registered therapist, to patients at risk of admission to hospital within 2 hours to support Urgent Community Response
To provide specific skilled support to the registered clinicians in providing technical therapy interventions to patients in a variety of settings

To use an evidence-based, person-centred approach in the planning, implementation, evaluation and monitoring of agreed interventions following guidelines and protocols to promote the functional fitness of patients in a way that is consistent with their abilities, preferences and cultural beliefs.

To be able to work flexibly over a 7/7 service rota
To work as a key member of the multi-disciplinary team carrying forward treatment plans and progressing rehabilitation goals for patients to enhance recovery
To communicate complex and sensitive information effectively to patients using skills of persuasion, motivation, explanation, negotiation and empathy. There may be barriers to understanding such as hearing loss, speech difficulty, anxiety, fear and depression
